Mob Heads Mod enables most vanilla mobs to have a chance of dropping their respective head, or skull, on death. The actual odds mimic the formula used to determine whether Wither Skeletons will drop a skull, and so boils down to 2.5%. The rates of mob head drops is impacted by the Looting enchantment, to the same degree as Wither Skeleton skull drops.
In addition, you can craft some of the special Mojang heads by combining their respective block/item and an enderpearl together in a crafting grid; the recipe is shapeless. (eg. Cactus Block + Enderpearl = Cactus Mini-block). These are wearable and placeable, just like any of the mob heads.
This mod utilizes the SkullOwner NBT tag of Heads, and so even after uninstalling this mod, the items will retain their appearance and functionality; however, upon placement and picking a head back up, it will lose its rename. Normally the mod corrects this, but names will revert on placement if you choose to uninstall the mod but keep the heads.
All heads are available from the Creative Menu as well.

A method to obtain individual player heads (besides the mob ones), using the new Mannequin block.
Craft a Mannequin block with the recipe:
Where ?W’ is a wool block, and ?S’ is a stick. It will face you when placed.
To use it, craft a Steve Head (four soul sand in a square), and rename the Steve Head to the player you want to create’s head in an anvil. Then right-click the Mannequin block with the renamed head. It will change to the name ?(name you entered)’s Head?. When placed, it will show that player’s head.

* To craft these Mini-blocks (heads), combine a sign, an enderpearl, and 0-5 pieces of paper in a crafting table (0 being Arrow (Up), 1 being Arrow (Down), 2 being Arrow (Left), and so on).
